Dear SHS Crew Members, Operations Team Members and Employees

Please familiarise yourself with the online Incident Report Form.

The IRF is part of our ongoing process to make our forms easily accessible to all SHS Crew Members and employees online.

The IRF has been specifically created for ease of use and to be available at all times and locations.

The IRF replaces the old paper version and requires no downloading, printing or scanning, is optimised for mobile use and is environmentally friendly.

It is important that ALL SHS Crew Members and employees use this IRF to report ALL Incidents, no matter how insignificant they may appear, for ANY incidents, near misses, injuries, potential injuries or damage to vehicles or equipment.

The IRF can filled in by ANY member of the SHS Crew or Employee or by any Client of Show Support.

Workers Compensation claims will require an IRF to be submitted with a Workcover Medical Certificate uploaded within.

The IRF is easily accessible on the Show Support website under the WHS heading: Show Support Website > WHS > Incident Report Form.

The paper version of the IRF is still valid and can be used if preferred. The paper IRF can be found on the Show Support Website > WHS > Resources.
